What happened
On August 6, 2024, Ronin Bridge was exploited for $12M (4K ETH + 2M USDC) due to an incomplete contract upgrade. The bridge manager was upgraded to v4, but the v3 initialization function, which set critical voting weights, was never called, leaving _totalOperatorWeight uninitialized and breaking vote-weight validation.
