What happened
StakeHound's 38,178 ETH became inaccessible when a 3-of-4 threshold signature key backup procedure failed. Fireblocks lost custody of 2 key shards during a machine failure in March 2021, and StakeHound failed to execute disaster recovery or verify the backup through third-party services. The keys became permanently unrecoverable.
